25a extend offer not functioning

Summary:

In 25a testing I am trying to "extend offer" but nothing is happening. Candidate still shows "Offer, Approved" after I confirm to extend the offer. Any one else experiencing this in a lower environment? We are scheduled to take 25a in PROD next Friday night.
