Tax Withholding page Error: "Invalid column index"

Receiving error message, "Invalid Column Index" when trying to add Default State and Local Withholdings to Employee's Calculation Card.

We are trying to add local tax withholding for an employee, but are getting the below error message. Does anyone know how to resolve? Navigation below.

We can reproduce issue in Production and Test.

Navigation:
1. My Client Groups>Calculation Cards>search for employee.
2. Select Calculation Card = Tax Withholding.
3. Scroll down to Regional section. Click the Add button to "Add Default State and Local Withholdings".
4. Error message appears.
