Receiving Duplicate notification for same item

We have multiple recruiters that are receiving multiple notifications for the same approval or items. When they select the link within the bell notification or email notification they receive and error message. This appears to be happening with notifications related to offers and we have not seen this error elsewhere. most recruiters have noticed this issue only since yesterday but one did mention receiving a similar error last week.
