Unable to see the notification under the bell icon

Summary:

We have utilized the 'Document approved or rejected' Oracle alert to have the notifications sent via email and worklist when the certificate is approved / rejected from benefits

Upon approving the document, we can see the notification sent as an email but NOT under the bell icon or the worklist. Am I missing any mandatory steps here?

Ran the processes Synchronize Notifications in Global Header &
Synchronize Notification Data from BPM to HCM, cleared the cache, but still cannot see the notification under the bell icon or worklist.
