We created an inspection in Quality Management for associates receiving services would evaluate it by responding four simple questions. The users find it complicated to switch from receptions to quality management module and we have a very low response rate.

Is it possible to create a satisfaction survey when receiving services?

Any other recommendation on how to manage evaluation of services received for suppliers would be greatly appreciated.
