Transaction approves before all approvers approve

We have a change assignment approval which goes to 9 approvers. Some of them are Representatives and Line manager. However when one of the approver with a HR Manager role approves, the whole transaction gets approved and the approver who is after the HR manager doesn't get any approval. We can see all 9 approvers in the transaction console when the transaction initiates and the moment HR manager approves the next approver in line disappears
