Touchpoint feedback template in alert composer

I have tried to trigger below feedback notification but its not working. Can anyone please let me know what action helps to trigger these two notifications?

When a participant completes the feedback requested by a manager for the direct report or requested by an employee directly, it triggers the highlighted template.
