Error in Alert Submission

Summary:

I am trying to create alert for a closed reconciliation as part of evaluation process by team of reviewer. 10 Mandatory questions are added to this alter type and set to mandatory for the evaluation reviewer to answer before submitting this alert. However, alert is submitted even when most of the questions are not answered hindering the purpose. Any advice or idea is greatly appreciated.
