Cannot inactivate duplicate supplier address. Receiving error.

Summary:

I went to add purchasing and RFQ (remit was already checked) to an existing address. I clicked on the existing address, check the boxes beside purshasing and RFQ, added the district number & phone numbers. Hit save and close. Went to the site and added purchasing clicked submit and and duplicate address was added. We do this all the time to during our daily maintenance, we've never had this happen.

Cannot reproduce in a lower environment.
