Add multiple questions on a requisition

We have a requirement where a demand for something is created using a requisition. In their current system the requisition can host several questions which need to be answered by the requisitioner. Based on the answers entered the buyer creates a contract. Is there a way we can add around 15 questions to the requisitions page. We were considering smart forms using information template but adding values in the Manage information template descriptive flexfields is difficult and not flexible as it has a small character limit. Is there a way we can achieve this requirement?
