OSMH - Unable to Register Instance After Migration

Symptoms:
An instance was migrated from OSMS to OSMH, but the instance is not seen in the OSMH Console and the Agent will not start in the OCI Console.
Cause:
- When attempting to register the instance in the OCI Console, the following is seen:
rpc error: code = Unavailable desc = connection error: desc = "transport: error while dialing: dial unix /var/lib/oracle-cloud-agent/tmp/plugin3798840359: connect: connection refused"
- The “osms-to-osmh discover managed-instance” command shows that the instance was not migrated (see Migrated column reads No):
