Manage Account Combination is not working in ADFDI as expected and showing as successful

In the "Manage Account Combinations" function, using the "Create in Spreadsheet" option, the Create Account Combinations in Bulk template is returning a row status of "Row inserted successfully" even when a row is not actually successfully created. After receiving this result, if the template is opened again, as if to enter new information, only then will failed rows show up.
