Trying to add Candidate as Prospect in a Requisition (Redwood) But Not Getting added

I am currently in Redwood UI - Trying to add Candidate as Prospect in a Requisition (Redwood) getting saved properly But candidate is Not Getting added and not getting displayed

The same is happening without any issues in the responsive UI, I have disabled redwood profile option - candidate is getting added and visible too

what could be the issue?
