Terminate Employment duplicating the exisitng reports step

Summary:

Hi, has anyone experienced the existing reports step duplicating in the Terminate Employment Process.

When we select Existing Reports on the info to include page and go through the process, it duplicates the existing reports step as seen in the screenshot and does not allow us to submit the process.

Your help is really appreciated.
