Approval History is blank for Change Personal Information:Biographical Info transaction

Hi Experts,

We had an issue with the transaction mentioned in the title. It seems to be working as expected couple of weeks back also. Suddenly, all the submitted transaction belonging to Biographical Info section seems to show like blank in approval history although in the transaction summary page it is being shown as forwarded to the recipient.

Summary of workflow:

Workflow is set to trigger to users with certain role if the duration b/w their hire and current date doesn't exceed 7. Now for biographical info transaction, in the approval history it shows blank. And customer is getting an error saying "Contact your help desk (FND-2)" error message when they claim and try to approve the transaction. The weird part is it is only happening for biographical info changes. The same issue we cannot replicate it in lower instances as well. Does anyone have any idea on the reason for this issue? is the something related to recent patch or
