Failed transactions in Oracle are not reporting to initiator

Hi All,

Failed transactions in Oracle are not reported to anyone. The initiators of the respective transactions are intended to be informed of the failed status so they can react and take the necessary steps.

We have enabled the creator in task configuration in the BPM worklist under notifications tab, but no notifications are triggering to the initiator.
