Checksum mismatch in ESS Tax Withholding When Adding a State and Opening the Form

Summary:

When we go to the self-service Tax Withholding page to add a new state (regional) entry and click on the link to the state tax form, occasionally a 'Checksum mismatch' error appears and the form does not load.

Steps to duplicate the issue:
Me > Pay > Tax Withholding > Click Regional > Click +Add > Choose 'State Taxes' > Choose a state (AL, AZ or MD) > Click on the state abbreviation under the Forms heading.
Result: Message on webpage appears as 'Checksum mismatch.Please re-login to the Application and try again'.

The error is not consistent for each state. We have seen one state form load without issue from one instance then received the error when loading the same state tax form from another instance.
