Redwood Change Assignment Not displaying Actions

In our Redwood Change Assignment transaction, we are unable to select an Action code for the question "What's the way to change the assignment?"

Content (please ensure you mask any confidential information):

In the When and Why section of the Change Assignment, we are able to select an Effective Date and an Action Reason code, but unable to select and Action code? Why would that be? We recently took a refresh of this environment but re-ran the ACL processes and the indexes. What more are we missing? This works in our other redwood environment without issue and our VBS changes are published to both our redwood environments so i would expect the transaction to look and function the same in these two environments.
