Default attachment category NOT working all the time

Summary:

Default attachment category NOT working all the time. Following setup is done at create_noncatalog_request page. Read only NEVER works. Many times page defaults to To Approver as expected but sometimes specially after adding to cart if users comes back to edit requisition line again it defaults to Internal to Requisition.

We tried with condition (like mode Enter or Edit Req) or without any condition. But no luck.
