Why are some learning assignments being displayed twice?

I'm seeing the same learning assignment displayed twice for some learners in some courses.

I'm increasingly seeing the same learning assignment displayed twice for some learners. At first I thought our IT dept had imported some duplicate records (Bypass Completed records), but it's also happening for learners who completed their enrolment in Oracle Learning (Completed records) - and also for learners who are only assigned and have not yet selected an offering (No Active Offering records).

It's definitely the same learning assignment being displayed multiple times, as evidenced by the fact that if I highlight one, the other is also highlighted; and if I edit one, the other is also updated. As well - they have the exact same assignment number.
