Trying to run a report on 'completed' for a new employee survey in Journeys

Summary:

I am trying to create a report on capturing a specific task in a journey. It's a New Employee survey (questionnaire), and no matter what I find on threads, the options I'm being told to select are not available to me. These are the only 2 subject areas I have, and I cant find anywhere in any of them that will allow me to choose this task, in this journey solely to capture completed or not.
