OCI: Oracle Linux Instance Fails to Boot with "Warning: /dev/mapper/ocivolume-root does not exist"

SYMPTOMS

Oracle Linux 8 Instance fails to boot with error below and dracut shell shows up:

[  OK  ] Reached target Initrd File Systems.
[ OK ] Reached target Initrd Default Target.
[ 208.902544] dracut-initqueue[617]: Warning: Could not boot.
Starting Setup Virtual Console...
[ OK ] Started Setup Virtual Console.
Starting Dracut Emergency Shell...
Warning: /dev/mapper/ocivolume-root does not exist
Warning: /dev/ocivolume does not exist
Warning: /dev/ocivolume/root does not exist Generating "/run/initramfs/rdsosreport.txt" Entering emergency mode. Exit the shell to continue.
Type "journalctl" to view system logs.
You might want to save "/run/initramfs/rdsosreport.txt" to a USB stick or /boot
