Item ruleset is not triggering change order for If condition query

Summary:

Ruleset type : Validation

Association Type: Item Class

If condition: ([Item].[Item Basic].[Approval Status] == "Approved") and true

Validation Condition: false

Expectation: Items in approved status, if any updates occur in any of the Specification attribute, the change order should trigger. This query was working fine some time back but now it is not working. Please suggest right query.
