Looking for Employee Salary and STIP report either OTBI or BIP report sql.

Summary:

I am looking for a report that would capture every salary and STIP eligibility percentage an employee had during the course of a year.  For some employees, that may be the same all year, for some – they have 1 change, and in rare situations, some employees have 2+ changes.
