Requisition has duplicated its own lines in My Requisitions front view in RSSP

Summary:

We have a requisition which has duplicated it's own lines 3 times in front view of 'My Requisitions'. But if you dig into the line details, life cycle and purchase it's one line,

What is the cause of this and what is the fix, is it a scheduled process?

We have proof of this as in our non production environment as of 26/09 it shows on requisition and line view as

In production as of 03/10 it shows, all we know is that it duplicated itself sometime in between these dates

User also confirms they raise from scratch as non catalog request. Many thanks
