Unable to create agent

in Recruiting, Opportunity Marketplace

When trying to create an agent via. the following navigation Setup and Maintenance >> Offering: Recruiting and Candidate Experience >> Functional Area: Source Candidates >> Task: Recruiting Agencies —>

Under the Agents section of the Recruiting Agencies page when we click the Add button and fill in the relevant details related to an agent, we are unable to complete the agent creation process. The following Error Message shows up as seen in the screenshot below -
