Is it possible to add a custom fields in My Goals redwood page?

We have a requirement for employee to acknowledge few things before Goals are submitted for approvals. Currently, DFF fields can be added in individual goals but our requirement is to have a mandatory fields for users to select before submitting Goals for approval.
