Approver not appear twice in PR approval notifications even though they were setup twice

Summary:

We have a requisition approval rule setup where an approver named "Carl " was set as a manager and a worker in the approval rule. When we go to create a requisition, we noticed that Carl did not appear twice in the approval notification as expected. Our expectation is the notification should have displayed Carl twice in the approval hierarchy since he was set up twice in the approval rule.
