Workflow approval from email is not updating in workflow system in oracle fusion

Summary:

Termination Approval workflow sending mail for approval. Once Approver approved the notification from email, it is not getting updated in workflow system. In approval hierarchy, still it is with same person.

Can anyone please help on this issue. Thanks in advance.
