Reverse Termination transaction Approval Getting Failed

Summary:

The admin has performed a Reverse Termination for an employee without encountering any errors during the process. Upon submission, the transaction was successfully submitted and it was approved by the workflow system

However, once it is approved by the workflow system, the transaction fails to complete and does not reflect as processed in the system.

Has anyone experienced a similar issue, or could you suggest what might be causing the failure after approval? Are there any known processes that could be impacting this behavior?
