Internal Candidate not moving from Offer-Accepted to HR state

An internal candidate accepted her offer for a requisition but remains stuck in the Offer – Accepted phase/state. Recruiters attempted to manually move her to HR Processing, and the system displayed a success message, but the phase/state did not update. No error messages appear.
