Unable to create PO from Approver PR

Summary:

Hi Team,
We have an approved requistion. We tried to create purchase order from process requisitions.
We searched with PR from approved requistion, then click on add to document builder then it show Create PO small screen.


Once we click on OK it getting disappeared. Its not creating an order.
Unable to understand the issue?
