SimulateConcurrentUsage Ad Hoc Grid function - getting EPMA-1 error

We have been trying to use simulateconcurrent usage to open an ad hoc grid in Excel with no success.

We have looked at KB76155 and put the UserVarMemberMappings.csv in the named zip file referenced in the epmautomate simulateconcurrent usage command. Attached are the logs, the simulateconcurrent usage command and the zip file the simulation is running. The logs read the following error: EPMAT-1:Invalid contents is available in a file identified in the requirement.csv file. However, we believe we have all the correct files in the zip, requirements file, input files, options file, and the UserVarMembersMapping.csv. We want to know what we are missing or is this an issue with the command?
