Questionnaire loaded via HSDL template is not showing up in the UI

Hi Team,

I tried to load sample record for the Questionnaire Template and Questionnaire through HSDL. After uploading the data, in the HSDL template, it showed that the record has been uploaded successfully, however it is neither showing up in the UI nor in the recon report. While loading the data I didn't get any error as well. Please note that I was able to load the Question successfully and its visible in the UI as well. Attaching below the HSDL templates for the reference. Please let me know how to resolve this.
