Odd error when trying to add to cart.

We have noticed a few times that a user will get the following error when trying to Add to Cart in Redwood Requisitioning.

When this happens the user can not do anything until they resolve the issue.

What we believe is the issue is a Canceled requisition is trying to add itself to the new request.

Has anyone had this issue?

our temp solution is to reassign the corrupt cancelled requisition to a holding account till we can figure out what the issue is.
