Issue With Notification Templates - Logo Breaks Approve, Reject, and Request Info Buttons

Editing the Logo in the Workflow Notification Subtemplate referenced in other notification reports e.g. Journal Approval Notification Report causes the approve/reject/request information buttons to break.

At the top of the Journal Approval Notification Report, it references the following subtemplate.

I customize and edit that template, put my logo on page 3 where it instructs to e.g.

I reupload the template to the custom location i.e. Shared Folders/Custom/Common Content/Templates. The logo starts to appear on the notification, but for some reason the Approve, Reject, and Request information buttons break even though I never touched that component of the subtemplate as I can see the FusionActionLinks also exists in the subtemplate and is referenced in the Journal Approval Notification Report RTF. I also never touched the Journal Approval Notification Report RTF either.
