Interview Feedback Forms Showing Duplicate Text Boxes

Hello! We have been noticing on our Interview Feedback Forms that questions we have listed as "text" responses are all of a sudden showing multiple text boxes. We do not have "comments" turned on for them. These forms are still displaying in Responsive due to a bug that we found on the Redwood forms. Has anyone else experienced this? Yesterday, our recruiter opened one and it showed four text boxes, but the next one she opened had two! Thank you!
