AI agent setup to read question content from the file attached to the tool

Hi, I am trying to setup an AI agent which is supposed to read question content from the file attached to the tool. The idea is the potential suppliers, during supplier onboarding process, will access this AI agent and answer the questions the AI ask. Based on their response to the questions, the AI agent will send the response summary to Approver for review and approval.

I am trying to setup a custom AI agent with a custom tool having the file containing the questions. When invoivked the AI agent does not read the questions from the file. Not sure what is the issue here. Has anyone tried similar thing?
