Transaction has failed even its approved by all Approvers.

Summary:

Transaction Resign from employement is initiated by an employee which went through all the heirarchy for an approval and everyone have approved it. But when the last approver HR approves it, Its failing with an error.

I have attached logs and detailed report in the attachment. Can anyone please help in understanding why it has failed?
