Case Management - New Action Plan template not visible in Cases

Hi All,
We are implementing Case management for our client and referring to Opt Out of Case Management implementation guide.
We have created some actions and actions plans templates via setup and maintenance :-
When we create a new case, this template is not visible in action plans also actions are not visible either. Can you please assist how can we attach the templates to cases.
We have assigned Case manager and Case worker roles to our users but still it is not visible, we have also run the adaptive search.
Case :- No values in drop down
