Action Descriptions not working within Action Plan

Summary:

Within Case Management, We have created an Action plan with several Actions that each have a description. Within the Action Plan, the action descriptions are not appearing, instead, it is a blank area for the case worker to type text. I thought this was supposed to be a description of the action they need to take?

Within the Action - we have configured the description of what this action entails. This is supposed to be instruction for the case worker on what needs to be completed n this task.

But, once we go into the actual Action Plan with the Case, each Action Description does not carry over, instead…it is blank space for the case worker to type their own description. Am I mis-understanding how this is intended to be used? Thank you in advance for any help that you can provide!
