How can I add custom fields to the Help Desk Request Details

I want to add custom fields for the Agent when a service request is received. For example we want to add a field stating "Payroll impact" - YES/NO. This is something only the agent should add to the request.

I have created the fields via App composer, when I try to open the page below in VBS I come to the Parent Page.
I need to edit and add the fields to the page below.
