Learning Assignment Reconciliation Not Impacting All Learners After Offering Update

There is an issue observed with a Learning Item update not impacting all learners associated with a Learning Assignment.
For example, when an administrator updates a required offering by adding a new activity or modifying the description, and then performs reconciliation, the system shows the impact for only one learner or sometimes no learners at all.
Kindly help identify what could be causing this behavior.
