Report for Journey Checklist task is sending the wrong candidate details

I've created a report with a template of candidate details in it for a Journey Checklist task for a task name : Arrange Laptop and System Access , the report sharing the wrong candidate details , it's taking a top row in the BIP report and sharing those details, please review the query and let me know what changes i should make?
