Onboarding Task Terminated

Hi -

Curious if anyone has any insight around tasks being terminated or canceled by candidates in Onboarding/Transitions? We have a couple examples of this happening on our personal information task and Equifax I-9 task. The onboarding process shows this as canceled for I-9 task and terminated for the personal info each completed by the candidate. This only appears to happen to one recruiting user or specific onboarding process, but I can't pinpoint what's causing it. I had opened a ticket previously in regards to the I-9 cancelation and support stated it appeared to be a one off
