Documents missing in Agent and Agent Team but exists in the Document Tool

Summary:

Hi, I uploaded 38 PDF documents to the Document tool - lookup_employment_lifecycle_policies. They all are published, are under 2 MB and show up when I look at the Tool. However, I only see 24 files in the Agent, randomly missing the pending 14 files. I ran the below processes but nothing seems to help-
