How to Track Lead Source with One Eloqua Form

Can a Single Eloqua Form Be Used on Multiple Web Pages with Query String Tracking to Identify the Source Page?
We have an Oracle Eloqua form that the business would like to embed on two different web pages:
- Golf Page:
- Sports Turf Page:
The requirement is to use the same Eloqua form on both pages while being able to identify which page the lead originated from after the form is submitted.
Is it possible to implement this via query string parameter (for example, passing a value such as source=golf or source=sports) and mapping it to a hidden field in the Eloqua form.
