Self-Paced Learning Video Shows a Not Passed Status for a Learner

Summary:

We recently assigned a self-paced learning item, which is a video, to a group of our learners. After looking at the completions on the admin side of things I noticed that one of the learners has a status of "Not Passed." There is no quiz, assessment, or any other additional completion requirement as part of this assignment other than the video itself. Has anyone ever encountered this issue or have any insight as to what would cause this?
