Supplier change request report

We would like to build report which includes supplier profile change request information like change request number, Attribute 'change from' and 'change to'. Could you please suggest table that store this information. We know audit tables (like poz_suppliers_ ) store 'change from' and 'change to' but we are unable to link it to change request number.
