How to get the details of action items which are denied?

Need details of the action items which are denied along with their denied date

We have a requirement to review the details of action items which are denied along with the action item name, denied date and who denied the action item. I know from UI, there isn't a way to get the details of denied action items, would like to know how we can pull the details from backend tables using SQL queries.
